For the purposes of these Terms and Conditions, the term "Reward" is defined as one (1) reward from the National Wellness Network, one (1) Subscription to Autio, or one (1) $50 Live Event Credit from TheTixFix. Customers may qualify for a maximum of one (1) reward per opportunity, unless otherwise specified by Breezeline.
In order to qualify for the reward, customer must be a current Breezeline customer in good standing, and take a qualifying action as defined by breezeline. The qualifying actions may include but are not limited to:
Receive a customer appreciation email from Breezeline.
Upgrade your speed.
Get caught up on your bill.
took another seperate qualifying action as specified and confirmed by Breezeline and ultimately received a verified email and code from Breezeline
The reward steps to redeem are as below:
Customer takes a qualifying action as listed above.
Customer will receive an email from Breezeline containing a unique Reward code and driving them.
Customer will be driven to us.rewardsforall.com.
Customer chooses their reward.
Each customer will receive (1) reward per reward opportunity, unless otherwise outlined by Breezeline.
This promotion will run from 4/6/22 – 12/31/24. These dates are subject to change.
The final day to submit the form is 1/31/24.
Customer must redeem their Reward code on us.rewardsforall.com no later than 11:59 PM PT 1/31/24.
Customer who receive a Live Event Code must upload it on www.breezelinerewards.thetixfix.com by 4/30/24.
$50 Live Event Credit is good towards (2) tickets.
Must be 18 years or older to participate.
Only valid to legal residents of Connecticut, Delaware, Florida, Maine, Maryland, New Hampshire, New York, Pennsylvania, South Carolina, Virginia, West Virginia, and Ohio.
For the purpose of these Terms and Conditions, the term "Reward" is defined as one (1) wellness experience at a participating venue or partner. Available experiences vary from venue to venue and may include but are not limited to: Zumba class, Yoga class, Pilates class, gym trial memberships, personal training sessions, nutritional consultations and other wellness experiences depending on the participating fitness facility or independent location near you.
Taxes and gratuity are not included. Additional costs/fees for fitness props or equipment required or mandated by the venue for participation are not included. The voucher is not redeemable against booked parties.
All repeat visits will cost full price. Anyone accompanying the voucher holder must pay the regular price for their experience.
Customer must present the voucher upon arrival at the venue along with valid photo I.D.
Reward valid in the continental United States only (excluding Alaska, Hawaii, US Territories and Puerto Rico).
This Reward is subject to promotional availability and may vary by participating venue.
This Reward may be based upon advance appointment times. Some venues are seasonal and only open during certain periods. It is the Customer’s responsibility to determine the hours of operation of the selected venue. It is the Customer’s responsibility to call the venue for full details prior to redemption. The Customer using the voucher must follow the instructions provided in the voucher and accompanying email. The booking instructions and all other information stated within the voucher and accompanying email also form part of the terms and conditions of this Reward. Customer must present the voucher upon arrival along with valid photo I.D. The person whose name is on the voucher must be present at time of redemption. Failure to comply with the instructions or any other directives made in conjunction with this Reward may result in cancellation and/or a charge for the Reward. Under these circumstances, Reward shall be considered redeemed, voucher will be voided for use and participant shall be solely responsible for any costs or fees incurred as a result of their noncompliance.
Consult your physician and follow all safety instructions before beginning any exercise program.
Each venue may also have their own Terms and Conditions in addition to the terms of this Reward, which will be binding upon choice of venue. It is the Customer’s sole responsibility to comply with these Terms and Conditions.
This Reward cannot be used in conjunction with any other promotional offer or applied toward a package, series of sessions or classes.
This information is correct at the time of campaign launch, although the venues and TLC Marketing reserve the right to change details at any time. Each venue featured reserves the right to make the final decision of admission, according to the rules and regulations established at their venue. TLC Marketing cannot be held responsible for this decision.
TLC Marketing cannot be held responsible for any disagreements concerning availability and quality of experience. Any dispute must be settled directly with the venue concerned.
TLC Marketing cannot accept any liability for personal loss or injury at the featured venues. Any dispute must be settled directly with the venue concerned.
Void if transferred, sold, auctioned, photocopied or otherwise altered and where prohibited or restricted by law.
TLC Marketing shall not be liable for lost, stolen, damaged, misdirected or undeliverable Reward vouchers and/or redemption requests and will not issue or replace Reward vouchers in any of these events.
The Reward has no monetary value. This Reward is not transferable, transferable for sale, resale and is not redeemable for cash.
Voucher valid until date indicated on voucher.
Partner Code valid until date indicated on email with Partner Code and redemption instructions.
The Reward is subject to promotional availability and the value and types of Rewards will vary dependent on the geographic location and vendor fulfilling the Reward.
No credit, refund or extension for any unused Rewards will be provided.
Failure to appear at venue at time of reservation (if applicable), without giving venue prior notice, gives venue the right to refuse Reward. This may result in charge for Reward or refusal by venue to reschedule Reward. Under these circumstances, Reward shall be considered redeemed, Reward will be voided for use and participant shall be solely responsible for any costs or fees incurred as a result of their noncompliance.
TheTixFix has its own Terms and Conditions in addition to the terms of this Reward that are required to utilize this Reward and are required for and binding upon Reward redemption. It is the Customer's sole responsibility to comply with these terms and conditions.
Each Live Event Code must be used to purchase (2) tickets.
The use of the TheTixFix website and the content is used at the Customer's own risk. TheTixFix privacy policy can be found here
Customer must redeem their Live Event Ticket Reward code no later than 11:59 PM PT on 12/31/2023.
Additional costs/fees for products and services outside of the Reward are not included and any such fees are the sole responsibility of the Customer. Valid credit card or PayPal account will be required to pay any amounts that exceed the reward amount Reward amount.
The Reward may only be applied to those tickets sold by TheTixFix from the start of the promotional period until 12/31/23. Reward cannot be applied to any purchases outside of the promotional period or not in the TheTixFix then current inventory. The Released Parties do not guarantee the inclusion of any specific event tickets in TheTixFix inventory at the time of Reward redemption.
The Released Parties will not be liable for any personal loss, injury, or any disagreements concerning quality of services or products from TheTixFix and will not accept any liability for personal loss or injury. Any Dispute must be settled directly with TheTixFix.
Each Stadium, venue, etc... may have its own terms and conditions in addition to the terms of this reward, which will be binding upon choice of event. It is the Customer's sole responsibility to comply with these terms and conditions.
This Reward is not transferable, transferable for sale, resale and is not redeemable for cash or credit.
By participating in this promotion, you understand that Reward tickets purchased through thetixfix include their own Terms and Conditions that must be accepted prior purchasing. The Terms and Conditions include, but is not limited to, COVID-19 related event cancellations and postponement. To assure fans’ safety during these uncertain times, all tickets are subject to restrictions and requirements put in place by venues, teams, or government authorities as it pertains to proof of COVID-19 vaccination, proof of negative COVID-19 test, social distancing, wearing personal protective gear, age restrictions, or similar measures. If you cannot attend the event due to your failure or inability to comply with such requirements, you will not receive a refund. If the event is held without fans or reasonably similar seats are not available, you will receive a refund as if the event was cancelled.
Failure to appear at the venue/stadium at the posted time of the event may result in the event being closed to admittance. Under these circumstances, the Reward shall be considered redeemed and the ticket may be voiced for use. Further, specific venues may have to reschedule certain events and may do so in their sole discretion. In the event of such instance the Released Parties shall not be responsible for any ticket refunds or future use. All issues related to cancellation or rescheduling of any event are the sole responsibility of the specific venue. In all instances the Customer shall be solely responsible for any costs or fees required for entry.
Reward may require a credit card or valid paypal account.
Reward code may not cover the value of the reward.
You may be responsible for paying additional fees and taxes.
Void where prohibited or restricted by law.
Credit card may be required to redeem Reward.
Reward is subject to promotional availability and available while supplies last.

The recipient of the Reward must follow the instructions provided by the Promoter, TheTixFix, Dining Advantage® and Breezeline. For the purpose of this Reward, "Released Parties" includes the Promoter, TheTixFix, Dining Advantage®, Breezeline, and their respective parents, affiliates, partners, subsidiaries, agents, distributors and vendors.
Released Parties shall not be liable for lost, stolen, damaged, misdirected or undeliverable Reward Codes and will not issue or replace Rewards in any of these events.
No credit, refund or extension for any unused Rewards will be provided.
Promoter reserves the right, in its sole discretion, to replace the Reward with another of equal or higher value.
This Reward is issued by the Promoter located at 60 Hudson Street, Suite 1809, New York, NY. 10013, or at such other address as the Promoter may advise in writing. No inquiry or notice will be validly received by the Promoter unless mailed to the Promoter's address above by first-class mail, postage prepaid.
No exchange of the Reward is permitted under any circumstance.
Promoter reserves the right to change or modify the Terms and Conditions of the Promotion at any time during the term thereof. It shall be the Customer's sole obligation to check the Terms and Conditions during the Promotion and when the Customer redeems the Reward.
